ABSTRACT

Objective:To search for evidence of capacity management in patients undergoing extracorporeal membrane oxygenation support therapy, and summarize the evidence to provide evidence-based basis for medical staff to evaluate and manage the capacity of such patients.Methods:This study was an evidence-based nursing study. Based on the 6S evidence model, relevant evidence on patient volume management in extracorporeal membrane oxygenation support therapy was systematically searched for relevant evidence on patient volume management in extracorporeal membrane oxygenation support therapy from top to bottom, including UpToDate, the National Guidelines Library of the United States, the Scottish Interhospital Guidelines Network, the Medical Guidelines Network, the Extracorporeal Life Support Organization website, Cochrane Library, PubMed, CINAHL, Wanfang, China National Knowledge Infrastructure, and the Chinese Biomedical Literature Database. Based on the inclusion criteria, clinical guidelines, expert consensus, clinical decision-making, evidence summary, and systematic evaluation were selected for literature quality evaluation to extract the best evidence. The search period was from April 10, 2017 to April 10, 2022.Results:A total of 11 articles were included and 20 pieces of evidence were extracted, which were categorized into four categories: extracorporeal membrane oxygenation team composition and personnel qualifications, evaluation and monitoring, capacity management objectives, and capacity management measures.Conclusions:Based on a large amount of evidence of extracorporeal membrane oxygenation support for patient capacity management, this study can provide a reference basis for clinical workers to develop extracorporeal membrane oxygenation support treatment capacity management plans.

ABSTRACT

Objective:To construct a myeloid cell specific Clcn7-G763R mutant mouse model and characterize its phenotype.Methods:A mouse conditional knocked in p. G763R mutation in Clcn7 gene was constructed and bred with LysM cre mice to obtain osteopetrosis mice with myeloid cell specific Clcn7-G763R mutation. The differences of bone mass in mice with different genotypes were analyzed using Micro CT and the changes of histology were observed with HE staining. Osteoclasts were cultured and the expression levels of osteoclasts differentiation and maturation-related genes were detected by real-time PCR. The functions of osteoclasts were examined through bone resorption assay.Results:The body weight of homozygous mutant mice at 4 weeks old was reduced compared with the wild type mice [(12.000±1.666)g vs(15.630±2.314)g, P=0.021], with shorter femur length [(1.160±0.096)cm vs (1.300±0.082)cm, P=0.037]. Micro CT showed that bone mineral density of homozygous mutant mice was remarkably increased at 4 weeks old [(0.753±0.002)g/cm 3vs(0.143±0.034)g/cm 3, P=0.003], while bone mineral density of heterozygous mutant mice increased significantly at 8 weeks old [(0.236±0.021)g/cm 3vs(0.180±0.020)g/cm 3, P=0.030]. HE staining revealed increased trabecula bone volume in the mutant mice, especially in homozygous mutant mice with narrow bone marrow cavity and wider hypertrophic zone of chondrocytes. There was no significant difference in the number of osteoclasts between wild type mice and heterozygous mice in vitro( P=0.358), while total area of osteoclasts increased in heterozygous mutant mice [(3.590×10 6±0.911×10 6)μm 2vs(1.352×10 6±0.260×10 6)μm 2, P=0.043]. Impaired function of resorption was unveiled by bone resorption assay. There were no significant differences in the expressions of osteoclast differentiation and maturity-related genes including NFATc1, c-fos, Ctsk, and Acp5 between the two groups. Conclusion:A myeloid cell specific Clcn7-G763R mutation mice with impaired osteoclasts and increased bone mass is successfully constructed.

ABSTRACT

Beijing Union Medical College Hospital applied the standardized management theory, focused on establishing a scientific and standardized system management mechanism, and carried out the exploration of modern hospital management standardization system construction through systematic practice. With the application of the principles of simplification, unification, coordination, optimization and competition of standardization into the construction of modern hospital management system, the basic method for the construction of standardization system was proposed based on standard system management theory. It included the standardization of system architecture, system construction process and system management mechanism. Its purpose was to effectively address the problems of the lack of systematicness, practicality and sustainability in the construction of hospital system. Importantly, the value and vitality of a system depended on its sustainable development. According to the standard system management principle of system effect, structural optimization, orderly development and circular control, the optimization of hospital system could be promoted, which is worth further practice and exploration.

ABSTRACT

Colorectal cancer is the third most prevalent cancer in the world. Surgical resection is the preferred method for the treatment of colorectal cancer, assisted by chemotherapy, targeted therapy and other methods to reduce recurrence and metastasis. However, drug resistance is an important factor in postoperative recurrence and death. Long non-coding RNA (lncRNA) has been found to be involved in drug resistance of colorectal cancer. Therefore, it is of great significance to study the mechanism of lncRNA regulating drug resistance in colorectal cancer. This paper reviewed the progress of lncRNA in drug resistance of colorectal cancer.

ABSTRACT

Background and purpose:Suppression of apoptotic signaling pathways is an important factor in tumor cell resistance. Research on cell apoptosis will open up a new way of reversing drug resistance and tumor treatment. This study examined the effects of a novel naphthalimide derivative 8c on multidrug resistant colon cancer HCT116/L-OHP cells and explored the molecular mechanisms underlying the apoptosis induction. Methods: The anti-proliferative effects of 8c were detected by CCK-8 assays and the effects on apoptosis induction were examined by lfow cytometry. The mRNA expression levels of p53, Bax and Bcl-2 were measured by real-time PCR;The protein expressions of p-p53, Bax, Bcl-2 and Cyt-c were detected by Western blot. Results:8c (IC50=8.16 μmol/L) seemed to be more potent than amonaifde (IC50=28.37 μmol/L) against HCT116/L-OHP cells. 8c induced apoptosis on HCT116/L-OHP cell lines through intrinsic or mitochondria dependent pathway. The protein expression of phosphorylation of p53 at Ser-15 was increased, but the mRNA level of p53 did not increase in HCT116/L-OHP cells. Bax protein and mRNA levels were signiifcantly increased, and Bcl-2 protein and mRNA levels were decreased, suggesting an increase of Bax/Bcl-2 ratios. Meanwhile, 8c induced a substantial release of cytochrome c from the mitochondria into the cytosol in HCT116/L-OHP cells. Conclusion: 8c induced cell death signal by inducing the activation p53 phosphorylation which subsequently activated related protein expressions of apoptotic pathway, which may be an important mechanism of 8c on inhibiting proliferation of HCT116/L-OHP resistant cells. All the results suggested that 8c was a potent compound to be developed as an anti-tumor and anti-resistance agent for clinic application in the future.

ABSTRACT

Objective To establish the quality standard for Qingxin Anshen Soft Capsules. Methods Rhizoma Coptidis and Cortex Cinnamomi in Qingxin Anshen Soft Capsules were identified by TLC. Berberine hydrochloride and cinnamaldehyde were determined by HPLC. Results The characteristic identification by TLC was distinct and highly specific. The results of quantitative evaluation showed that berberine hydrochloride had the linear range of 0.6412 ?g~6.412 ?g(r=0.999 7,n=7),and the average recovery was 97.57 %(RSD=0.81,n=6);cinnamaldehyde had the linear range of 0.2148 ?g~2.148 ?g(r=0.999 8,n=7),and the average recovery was 97.70 %(RSD=0.88,n=6). Conclusion The method is reliable,accurate and specific. It can be used for the quality control of Qingxin Anshen Soft Capsules.

ABSTRACT

AIM:To stuudy the relationship of the constituents of Erxiang Zhitong Capsule(Rhizoma alpiniae officinarum,Rhizoma cyperi and Radix aucklandiae) and its pharmacodynamics.METHODS:The use of supercritical CO_2 fluid extraction as extraction for three herbs acted as experimental materials.The preparation composed of Rhizoma Alpiniae officinarum extract,Rhizoma Cyperi extract;Radix Aucklandiae extract(3∶3∶2) had obviously antiulcerative,antispasmodic and anti-inflammatory analgesic effects.CONCLUSION: From view of pharmacodynamics view,we gain ratio of components of Erxiang Zhitong Capsule better than that of classical prescription.

ABSTRACT

AIM:To establish the quality standard for Xianglian Zhixie Capsule(Radix Aucklandiae,Rhizoma Coptidis).METHODS:Radix Aucklandiae,Rhizoma Coptidis of Xianglian Zhixie Capsule were identified by TLC.Dehydro-costunolide,costunolide were determined by HPLC together.RESULTS:The characteristic identification by TLC was distinct and highly specific.The quantitative evaluation of dehydro-costunolide had the linear range of 0.217 4-2.174 ?g(r=0.999 9,n=6),the average recovery was 96.25%(RSD=1.37%,n=6).The quantitative evaluation of costunolide had the linear range of 0.199 2-1.992 ?g(r=0.999 3,n=6),the average recovery was 101.77%(RSD=1.23%,n=6).CONCLUSION:The method is reliable,accurate and specific.It can be used for quality control of Xianglian Zhixie Capsule.
